Organ donation and transplantation is not a new thing in Malaysia, but the willingness to become a registered organ donor among the public especially the students are still at a low level. The title selected is focusing on the problems faced by society where people still are in doubt the donation issue organ, and what more will sign up as donors. There are four objectives constructed, which are, to determine the relationship between knowledge of student about organ donation and their willingness to become a registered organ donor, to determine the relationship between family influence and student willingness to become a registered organ donor, to examine the relationship between promotion of organ donation to the willingness to become a registered organ donor among university student and to identify whether self-efficacy will affect the willingness of student to become a registered organ donor. The sample of 377 questionnaires had been distributed at University Teknologi Mara (UiTM), Shah Alam. The data were analyzed using SPSS 19.0. The result shows all the variables divided into two dimensions. The dimension of the independent variable was named as awareness, mindfulness, approach, approval, inspire and discourage. For dependent variable, the dimension was named as readiness and intention. In additional, capability and incapability are dimension for self-efficacy. However, mindfulness dimension are not reliable to this research and had been deleted. Test of multiple regression found that family is the most influences the willingness to become a registered organ donor among university student. Finally yet importantly, by using Hayes Process found that all of the dimensions had an interaction effect with self-efficacy. Means that, the relationship between the dependent variables and independent variables are moderated by self-efficacy.
